Pricing
Enrollment Fee: New members require a one-time non-refundable fee of $100 per person . This amount covers the cost of creating the patient record, medical account, and other administrative fees. There is a $200 max enrollment fee for families of 2 or more.
Membership Fee: The monthly membership fee will start at the time of Enrollment. The regular billing cycle will resume on the 1st of each month.
Cancellation policy: We understand things happen, and plans change. We hope to create a lasting relationship with our patients, but in the event that you need to cancel we ask for a 30-day notice. No penalties or fees for cancellation. If you decide to rejoin at a later date, you may be asked to pay a re-initiation fee.
Credit Card Surcharge: Effective July 1, 2024 new members will incur a 3% surcharge for all credit card purchases. Please use ACH payments to avoid this additional fee.
